胃酸常刺激食道 致癌率大增

現代人工作壓力大，生活作息不正常，抽煙、喝酒、吃甜食、喝咖啡、喝茶、熬夜、大餐吃到飽等，使「胃食道逆流」常伴作多人左右；常見症狀有心口灼熱(俗稱火燒心)、溢酸水、吞嚥困難、喉嚨痛、或喉嚨有異物感、聲音沙啞、慢性咳嗽等。最常發生在飯後、腹壓增加(彎腰或提重物)、平躺時。若不加理會，胃酸長期刺激食道黏膜將可能導致細胞變性而轉變成巴洛氏食道，導致食道癌機率是常人的四十倍。

高醫中醫部表示，使用中藥和針灸治療胃食道逆流相關不適症有數千年歷史，近年來也有許多臨床研究證實對緩解症狀、改善生活品質確有不錯效果。

二○一二年由日本學者發表的多中心臨床隨機研究顯示，由蒼朮、白朮、人參、茯苓、半夏、陳皮、大棗、生姜、甘草組成的複方，對反覆發作胃食道逆流的患者之相關症狀有改善效果。在針灸研究方面，二○○七年由美國學者所發表的臨床隨機試驗中指出，針刺膻中、中脘、內關、足三里、陰陵泉、太衝等穴對於一些反覆的逆流性的症狀，尤其是日間或夜間的心口灼熱感有明顯的改善效果，並可能有抑制胃酸分泌、促進胃食道的蠕動和減輕胃壁的過度敏感等作用。

急則治其標，緩則治其本。胃食道逆流急性發作時，服用西藥制酸劑控制症狀，平時用中藥調胃整腸，修復胃壁，進而治癒擾人的火燒心。

嬰髖關節脫臼 長大恐長短腳

嘉義地區近半年有三例髖關節脫臼新生兒，若無早期發現，長大易變成長短腳，或肌肉無法正常使力而跛行，家長須注意。

嘉義基督教醫院今天表示，胎兒長時間在母親肚子屈著小小身體，容易出現所謂「先天性」髖關節脫臼，發生率約為○‧一%。

嘉義地區近半年來，有三例髖關節脫臼新生兒，以低出生率來看，新生兒髖關節脫臼比例算高。

嘉基小兒骨科醫師陳俊和指出，嬰兒若在六個月大以前發現髖關節脫臼，只要以吊帶固定慢慢矯正，若已超過六個月大，必須徒手將脫臼關節復位，再以石膏固定，如果等到開始學習走路才發現，則須透過開刀治療。

陳俊和表示，嬰兒在六個月大以前因股骨頭尚未骨化，可以利用超音波進行髖關節脫臼篩檢，若超過六個月大可採用X光檢查。

陳俊和說，以往醫師都是利用徒手進行新生兒髖關節脫臼的篩檢，但因大臀肌肉層較厚，易造成誤診。

陳俊和說，新生兒髖關節的脫臼，若沒有早期發現治療，往後容易變成長短腳，或是肌肉無法正常使力而跛行，因此，嬰兒髖關節脫臼要早期篩檢，愈早發現、治療，效果越好。

甲狀腺功能過高 長者易憂鬱

一份荷蘭最新研究指出，甲狀腺功能略高，可能增加老年人憂鬱症風險。醫師提醒，甲狀腺分泌過多，在生理上會出現手抖、心悸、體重減輕等症狀，心理則會有焦躁、神經質等情緒，建議有這些情況的民眾，應就醫檢查。

董氏基金會指出，荷蘭Erasmus醫學中心的研究者針對1500多名、平均年齡70歲的老人進行調查，一開始這些老人並未罹患憂鬱症，但經過8年多追蹤，發現甲狀腺功能屬正常範圍、但比平均值活躍的人，較有可能發展出憂鬱症。該份研究日前已刊登在《臨床內分泌與代謝期刊》。

新光醫院精神科主任王恩南表示，甲狀腺分泌量會影響情緒已獲醫學證實，在憂鬱症患者裡，有5％的患者罹病成因與甲狀腺有關，且過低或過高都不好，過低時會有身體疲勞、抑鬱、行動遲緩、無法集中精神等問題；過高則會有容易流汗、手抖、神經質、焦慮等症狀，民眾若出現類似症狀，都應盡速向醫師求助。

林口長庚醫院精神科主任劉嘉逸表示，造成老年人憂鬱的原因有很多，可能是身體疾病、身體功能退化或是藥物影響等，建議家中有長輩者，應隨時留意長者情緒，若長輩突然對原本有興趣的事物興趣缺缺，或情緒低落持續兩周以上，就應安排就醫。

董氏基金會心理衛生組主任葉雅馨說，老人憂鬱症常與其他身體疾病有共病現象，容易被忽略，要察覺老人情緒變化，需與他之前和現在情形作比較，若長輩明顯變得沉默、不快樂、常常流淚，在睡眠、飲食上有大改變，都應在就診時和醫師討論。
